Welcoming Spring with your Skokomish Community Health Representatives
Our team is getting more involved with outreach and events around the community! Please reach out to us if need of a transportation to and from doctor’s appointments, Medication pick-up and drop-off on Mondays, Wednesdays, and Fridays, and navigation through social services department. Call the clinic to schedule the transport or medication pick-up we are here to help.
SDPI Higher Quality Care Update
The Skokomish Special Diabetes Program for Indians (SDPI) would like to remind and encourage Skokomish community members who have been diagnosed as diabetic or prediabetic to schedule their quarterly lab testing, and setting up automated data reporting between your CGM equipment and the Wellness Center.
