Guidance for Skokomish Tribal Members Seeking Medical Care at Mason General
When you or a loved one who is an enrolled Skokomish Tribal member or eligible for Purchase Referred Care (PRC) are admitted to Mason General Hospital, we at the Skokomish Clinic ask that you present with your tribal identification card and state that your primary care provider is located at the clinic; doing so allows for Mason General’s case management staff to communicate to us that you are receiving care there.
Updating Emergency Contacts
Those community members who access services through the Wellness Center must double-check with the clinic’s staff to ensure that we have the correct and most up-to-date emergency contact information in our system. Doing so goes a long way to bolster the clinic’s ability to communicate effectively on your behalf, particularly in times of crisis.
SDPI Higher Quality Care Update
The Skokomish Special Diabetes Program for Indians (SDPI) would like to remind and encourage Skokomish community members who have been diagnosed as diabetic or prediabetic to schedule their quarterly lab testing, and setting up automated data reporting between your CGM equipment and the Wellness Center.
