Meet Your Community Health Representative Team

A Community Health Representative (CHR) is an individual who serves as a vital link between healthcare systems and underserved communities. Typically originating from the community they serve, CHRs possess a unique understanding of the cultural, social, and economic factors that influence health behaviors and outcomes. Their role is not merely logistical but deeply rooted in trust and rapport-building, enabling them to foster communication and understanding between community members and healthcare professionals. This intricate connection often empowers communities to engage more actively with health services, thereby improving public health initiatives and outcomes.

About:

The CHR program is based on the concept that indigenous community members, trained in the basic skills of health care provision, disease control, and prevention, are best able to gain community acceptance and make use of limited health care resources as efficiently as possible.

Values:

The Community Health Representative (CHR) program aims to provide quality health promotion and disease prevention services to American Indian and Alaska Natives using well-trained CHRs, who are integrated into and partner with patient’s health care team.
