Special Diabetes Program for Indians
The Special Diabetes Program for Indians (SDPI) is an Indian Health Services (IHS) funded program that provides funds for diabetes treatment and prevention services to indigenous communities across the United States. Skokomish SDPI provides education opportunities, care coordination, and assistance with acquiring diabetes management materials to Skokomish community members diagnosed with diabetes and prediabetes. These services are integrated into the clinic's case management and care coordination systems. Participation in SDPI programming and services is voluntary, and the form for referrals and self-referrals can be found on this page.

SDPI Coordinator/Wellness Case Manager
My name is Sasha! I am the Wellness Case Manager for the Skokomish Wellness Center, and I also coordinate the Skokomish SDPI (Special Diabetes Program for Indians). I am available to all Skokomish tribal/community members as an advocate in accessing social and medical services, without judgment or reservation. My hours are 8am to 5pm, Monday through Friday.
