What is National Healthcare Decisions Day? 

National Healthcare Decisions Day is a day to put in writing your wishes for medical care just in case you are too sick or injured to speak for yourself. This allows you to get the care you want and easier for your family, so they do not have to guess.

 

What are the steps to complete the paperwork?

Think about what matters most to you in a medical emergency.  Have a conversation with family and friends about your choices.  Write down what treatments that you do or do not want. Ask Skokomish Wellness Center for the paperwork to complete.  Your provider is a great resource to answer any of your questions or concerns.
